Nursing Approach to the Client with Motoric Bowel Disorders
Nursing Assessment
1. Assess for personal and family history, congenital defects, previous abdominal trauma, autoimmune and allergic states, medications usage, previous bowel infections or disorders
2. Collect information of complaints that brought client to the hospital.
3. Obtain history of onset and progression of symptoms.
4. Obtain information of signs of bowel inflammatory process, obstruction, or GI bleeding
5. Perform complete physical assessment including vital signs, signs of dehydration, GI bleeding and acute abdomen, stools, signs of diarrhea or constipation, hydration state of the client
6. Assess diagnostic tests and procedures for abnormal values
Nursing Diagnosis
1. Increased risk of dehydration, intoxication, GI bleeding, electrolytes disturbances, blood clotting disorders, related to diagnosis
2. Increased risk of peritonitis due to perforation of the bowel wall, related to inflammatory process and bowel wall destruction
3. Body metabolic abnormalities due to bowel disease
4. Pain and stool frequency disturbances , related to diagnosis
5. Appetite changes and weight loss due to symptoms of the disease
6. Changes in bowel habits, related to diagnosis
7. Anxiety related to the symptoms of disease and fear of the unknown
Nursing Plan and Interventions
Goals
1. Increase comfort and rest
2. Promote adequate nutrition and fluid intake
3. Prevent avoidable injury
4. Then surgical intervention prescribed, prevent postoperative complications
5. Relief or diminish symptoms
6. Decreased anxiety with increased knowledge of disease, it treatment, way of prevention and follow-up
Interventions
1. Assess, report , and record signs and symptoms and reactions to treatment
2. Monitor fluids input and output closely
3. Administer appropriate medications as prescribed, monitor for side effects
4. Monitor client’s vital signs and signs of possible hemorrhage and perforation closely
5. Monitor client’s stools and laboratory tests results (CBC, electrolytes, pH, Hb levels) for abnormal values
6. Provide prescribed diet
7. Administer IV therapy and blood transfusions as prescribed
8. Prepare client and his family for surgical intervention if required
9. For client after surgical intervention provide postoperative care and teach about possible postoperative complications
10. Provide emotional support to client and his family, explain all procedures to decrease anxiety and to obtain cooperation
11. Instruct client regarding disease progress, diagnostic procedures, treatment and its complications, home care, daily activities, medications usage, diet, restrictions and follow-up
Evaluation
1. Reports increased comfort, decreased anxiety
2. Verbalizes absence of pain and bowel discomforts
3. No evidence of hemorrhage and acute abdomen
4. Maintains stable vital signs, fluid balance, body weight and nutritional state
5. Laboratory tests results shows no abnormalities
6. No postoperative complications, or treatment complications
7. Demonstration of understanding of disease progress, diagnostic and treatment procedures, prevention, and need for follow-up
